You will have to obtain treatment from a physician who is on the Workers’ Compensation Division’s Approved List of Doctors (ADL). The ADL can be found online at www.tdi.state.tx.us.
You must also provide notice to your employer within 30 days of your injury or the diagnosis of your illness. A completed copy of the DWC-41 must be submitted to the Division office within one year of diagnosis or injury.
